If no author is given, use the title of the document or headings
Do not include initials or full names of the author or author’s first name(s)
When using a narrative, write out the authors or author’s surname(s), or use the title of the document
When not using a narrative, write out the author or authors’ surname(s) and use an ampersand [&]
